- Secure provider portal: AscensionHealthPlan.org
- Phone verification: 888-711-0494 (Monday through Friday, 7 a.m. – 7 p.m. CT)
- Self-serve Interactive Voice Response (IVR) system: for after-hours inquiries
Key information needed:
- Member ID or last four digits of SSN
- Member date of birth
- Date of service
Tip: Always verify eligibility on the date of service to avoid retroactive terminations or benefit changes.
