To find an in-network provider, use the Provider Directory. You can search by provider name, specialty or location to find care that meets your needs.

You may receive a bill if you are responsible for a deductible, copayment, coinsurance or services not covered by your benefit plan. Review your Explanation of Benefits (EOB) to compare the provider's bill with the amount you may owe. If you have questions, contact your 24/7 Member Services team at 888-711-0494.

An Explanation of Benefits (EOB) is not a bill. It is a statement that shows how your medical claims were processed and the payments made by your health benefit plan. The EOB includes information such as the date of service, provider name, amount billed, allowed (negotiated) amount, plan payment, deductible, copayment, coinsurance and any amount you may owe. Review your EOB carefully before paying a provider bill.

You or someone you name in writing to act for you (your authorized representative) may file an appeal. Complete, detach, copy and send in the Appeal Filing Form found on your Explanation of Benefits (EOB) within one hundred eighty (180) calendar days from receipt of notification of the denial.

To access your pharmacy benefits, visit Judi Rx (coming soon). There, you can access information about your prescription drug coverage, view participating pharmacies and manage your pharmacy benefits.

You have access to 24/7 virtual urgent care through LiveHealth Online.
