Luminare Health is the new benefit administrator for your employer's health benefit plan. We process your claims, answer your questions, and manage other aspects of your health benefits. We wanted to take this opportunity to welcome you and provide you with some helpful information.
Navigating Your ID Card
You and all your eligible, covered family members will receive member ID cards in the mail. When you receive your ID card(s), please keep them in a safe place and make sure to take them with you the next time you go to the doctor or pharmacy. These ID cards include important information about your benefits that your doctor’s office will need to know.

Front of Medical ID Card
The Member tab displays...
- Your employer’s name, group number, and plan type
- Your name and unique ID
- All covered dependents
The Medical Plan tab displays...
- Contact information for your Primary PPO network
- Medical copay amounts
The Pharmacy Plan tab displays...
- Contact information for your Pharmacy Benefits Manager
- Rx Bin, Rx Group, and PCN numbers
- Pharmacy copay amounts
Dependent Coverage:
- All dependents 18 and older will receive an ID card in their own name
- ID cards for dependents will always show “No” for dependent coverage
- ID cards for the plan subscriber will show “Yes” if their plan covers dependents under age 18
- If the subscriber has no dependents or if all covered dependents are over age 18, their ID card will show “No”
