Luminare Health is the new benefit administrator for your employer's health benefit plan. We process your claims, answer your questions, and manage other aspects of your health benefits. We wanted to take this opportunity to welcome you and provide you with some helpful information.
Navigating Your ID Card
You and all your eligible, covered family members will receive member ID cards in the mail. When you receive your ID card(s), please keep them in a safe place and make sure to take them with you the next time you go to the doctor or pharmacy. These ID cards include important information about your benefits that your doctor’s office will need to know. Remember, you can also access your ID card digitally.
The information on your card makes it easy for you and your doctors to quickly confirm important details about your plan.

Front of Medical ID Card Displays:
- Your employer's name or logo
- Your name and unique ID
- Your medical group number and plan type
- Rx Bin, Rx Group, and PCN numbers
- Medical deductible and out-of-pocket amounts
- Medical copay and coinsurance amounts
NOTE: All dependents will receive an ID card in their own name.
