Easy-to-read sections on both the front and back of the card make it easy for you and your providers to quickly confirm key coverage details.
Front of Medical ID Card
The left side of the card displays:
- Your unique member ID
- Your plan type, employer’s group number, plan code, and more
- Rx Bin, Rx Group, and PCN numbers for pharmacy use
The right side of the card displays:
- Your member portal URL: wellstarhealthplan.org
- In- and out-of-network employee and family deductibles
- In- and out-of-network employee and family out-of-pocket max amounts

Back of Medical ID Card
The left side of the card displays:
- Payer ID for electronic claims submission
- Mailing address for claims submission
- Prior authorization information
The right side of the card displays:
- Your network URL: anthem.com
- Your member portal URL: wellstarhealthplan.org
- Contact information for your member concierge team
- Contact information for your providers to verify eligibility and benefits and submit for pre-certification
- Additional contact numbers for your plan
