Easy-to-read sections on both the front and back of the card make it easy for you and your providers to quickly confirm key coverage details.

The front of the card displays:

  • Your name and your employer's name
  • Your unique member ID and group number
  • Your member concierge phone number
  • Your member portal URL: myPromiseHealthPlan.com
  • In-network (Promise Health Plan Network) and secondary network (PHCS) copays for different types of care visits
  • In-network (Promise Health Plan Network) and secondary network (PHCS) deductibles and out-of-pocket maximums
  • Name of your pharmacy plan and phone number
  • RxBin, RxGroup, and RxPCN numbers for pharmacy use
  • Your pharmacy costs
