Get to Know Your Card
Easy-to-read sections on both the front and back of the card make it easy for you and your providers to quickly confirm key coverage details.
Front of Medical ID Card
The Member tab displays:
- Your customer service phone number (at the top)
- Your member portal URL: myMemorialHealthPlan.com (at the top)
- Your employer's name and group number
- Your unique member ID
The Pharmacy Plan tab displays:
- RxBin, RxGroup, and RXPCN numbers for pharmacy use
- Your pharmacy plan's website and phone number
- Your retail and mail order costs
The Medical Plan tab displays:
- Your out-of-network network's number phone and website
- Copay amounts for different types of care visits
- In- and out-of-network employee and family deductibles
- In- and out-of-network employee and family out-of-pocket max amounts

Back of Medical ID Card
The Medical Claims tab displays:
- Payer ID for electronic claims submission
- Mailing address for claims submission
- Your out-of-network network's number phone and website
The Eligibility & Benefits tab displays:
- Contact information for your providers to verify eligibility and benefits
The Care Management tab displays:
- Contact information for your providers to submit for pre-certification
